Please include the following information:

1. A brief description of what you would like help with.
2. Your insurance situation. (Even though I am not in-network on any insurance panels, knowing what insurance you have will help me guide you regarding the money aspect of this process.)
3. General times of day that would be good for me to call you, with your return phone number.
4. Please let me know if you do not want me to leave messages at any telephone number you give me.
