Do you take my insurance?

I am not on any insurance panels anymore. I resigned from Anthem Blue Cross in February 2020. Therefore, I am out-of-network for all insurances (see below for special circumstances with Medicare). However, if your insurance is a PPO or you have other medical funding, you should be able to recover some of the money you pay for my services. I Opted-Out of Medicare. This means that if you have Medicare for any reason (including your age, if you are a dependent of someone who has Medicare, or if you have disability benefits), you and I can make our own arrangements; however, neither you nor I can bill Medicare for my services and we have to sign a specific and separate contract.
