Do you take my insurance?

I am not on any insurance panels anymore. I resigned from Anthem Blue Cross in February 2020. Therefore, I am out-of-network for all insurances (see below for special circumstances with Medicare). However, if your insurance is a PPO or you have other medical funding, you should be able to recover some of the money you pay for my services. (See Can you help me figure out my insurance? and What are your fees?)

I Opted-Out of Medicare. This means that if you have Medicare for any reason (including your age, if you are a dependent of someone who has Medicare, or if you have disability benefits), you and I can make our own arrangements; however, neither you nor I can bill Medicare for my services and we have to sign a specific and separate contract. (See What about Medicare?)
